- Citation: "During the operations from July 19-22, 1918, he displayed great bravery and fine zeal in exposing himself with profound contempt for danger in the most dangerous zones of the battlefield in search of the wounded whose evacuations he effected."
- "He was conspicuous for his gallantry and devotion to duty during the offensive action of July 18-22, 1918. During these days with utter diregard for his personal saftey or welfare, eh continually exposed himself to the most heavily shelled areas of the battlefield searching for, locating, and expediting the removal of the wounded."
- Citation: "For gallantry in action near Soissons, France, 18-21 July, 1918, in searching for and locating wounded under heavy shell fire."
- Belgian Citation: Similar to the American for same deed.
- Citation: "For distinguished and exceptional services as director of ambulance companies in teh operations of the American Expeditionary Forces. In testimony therof and as an expression of appreciation of his services I award him this citation." (Signed) John J. Pershing, Commander-in-Chief, Awarded May 16, 1919
