According to the Virginia War History Commission Questionnaire completed by his father Rev. Joseph Lewis on 23 May 1924, Thomas entered service on 13 December 1917. According to Rev. Lewis, he wrote that "My son was taken with pneumonia and taken to Camp Custer while suffering with the disease in zero whether from Jefferson Barracks, where he lived about a week. He died Feby. 1, 1918."
